
1. The parent/court appointed legal guardian will report to the school and sign SBO Form #13. At this meeting the parent/court appointed legal guardian will present pictured identification to validate his/her authorization to withdraw the child. This form must have a forwarding address.
2. The student is given SBO Form #584, which is taken (generally by the student) to each of the child’s teachers for the teacher’s assignment of a withdrawal grade and indication that the child has returned the school materials (textbook, etc.) associated with that class. Further this form must have the signatures of each support professional in the building, such as the nurse, bookkeeper, librarian, etc.
3. The completed SBO Form #584 is returned to the Guidance Office and the withdrawal process is completed and the student is issued a copy of this form to accompany him/her to the new school.
4. If a child is absent for 15 consecutive days and efforts to ascertain the reason for the absences via contact from the school and/or contact by the school social worker, the child is withdrawn as a W-9 and continued follow-up will take place. Examples of follow-up will include a Truancy Referral Packet being processed.
Withdrawal Codes
W1 – Pupil promoted or transferred to another room in the same school
W2 – Pupil promoted or transferred to a public school in the same county or city
W3 – Pupil transferred to a non-public school or a school in a foreign country which is not operated by our own government
W4 – Pupil transferred to a public school in another county or city within the state or to a State-operated institution or hospital within the state
W5 – Pupil transferred to a public school in another state (This includes any school operated by the Federal Government.)
W6 – Pupil withdrawn because of death
W7 – Pupil graduated
W8 – Pupil withdrawn for other reasons and not entering another school
W9 – Pupil withdrawn after being absent 15 (fifteen) consecutive days who is expected to re-enter school. At the end of the school year, a W9 pupil who has not returned to school should be coded and counted as a W8.
